Twisted Blot differs drastically from his standard Toon form. His overall coloring is darker and his mime-style makeup is smeared. His eye now features a red sclera with a black iris, framed by a dark shadow across his face. His pupil consists of a distorted red ring encircling an irregular red shape. The back of his head is elongated and stained with Ichor, tapering to a sharp point that drips Ichor, somewhat reminiscent of the back of Twisted Shrimpo's head. The drip marks on his face end in Ichor stains as well. His hands have grown larger and developed claws, with Tapes wrapped around them.
Twisted Blot is a stationary Twisted who does not roam, functioning similarly to Twisted Razzle & Dazzle and Twisted Rodger. He plunges his hands into the ground and summons giant Ichor arms at various points on the map, controlling them whenever his eye is open. These arms lash out at any Toon that ventures too close. After a period, the arms retract and Twisted Blot rests with his hands raised for roughly ten seconds before summoning them again at different locations. Unlike Twisted Razzle & Dazzle and Twisted Rodger, his attacks are not conditionally triggered and his arms will strike any target within range automatically. The arms can also mislead inexperienced players by making beckoning hand gestures, as though luring them closer with a reward, though the hazard is usually apparent enough that most players will not be fooled.
Twisted Blot's main body is entirely passive. He sits stationary on the floor, periodically waking up to relocate his two Ichor arms. Standing near his body causes no harm, but unlike Twisted Glisten, being close to him while he is awake will not yield Research, Tapes, or Event Currencies.
While his body is harmless, his summoned arms are aggressive. Getting too close to either arm triggers an attack attempt that deals 1 point of damage on contact. The arms telegraph their strikes with a brief windup, giving you a small window to exit their range. Staying near the edge of an arm's reach makes dodging easier. Both arms have limited range, so escaping should be straightforward in most cases. Be aware that Twisted Blot can summon an arm directly on top of his own body, making it risky to idle near him. After an arm slams down, it is briefly stunned for about 1 second, allowing you to walk past safely. If an arm is mid-attack but gets relocated, it will not deal damage.
Encounter Research, Tapes, and Event Currencies are awarded by his arms rather than his body. Earning the 5% encounter Research and Tapes may require entering an arm's danger zone. One approach is to walk into the very edge of the arm's range to trigger both the attack and the encounter credit, then immediately sprint back out. This is more difficult with slower Toons.
His arms can create serious logistical problems. They can temporarily seal off entire corridors and even block Machines, forcing Toons to wait for them to despawn. If another Twisted is chasing you and an arm blocks your only exit, you may be forced to take a hit. Arms can also appear inside a distractor's loop, disrupting their route or forcing them to relocate. Since arms can spawn on top of players, idling on Blot Floors is inadvisable. If you notice arms beginning to materialize near you, evacuate immediately. Sometimes waiting for an arm to despawn is the safest option. Like Twisted Sprout's tendrils, moving quickly along the edge or dashing as Flutter or Rudie can let you slip past without taking damage.
Arms become especially dangerous during Blackouts since they blend into the dark environment, much like Twisted Sprout's tendrils. Move carefully to avoid stumbling into them when visibility is limited. Communicating arm positions to teammates can prevent nasty surprises. Equipping Spare Bulb improves arm visibility, though it is a very situational Trinket choice given that Twisted Blot is a Rare Twisted with low spawn frequency.
Maps with dead-end Machines become particularly hazardous when Twisted Blot is present, as his arms can block entry or exit from those alcoves. If you want to work on a Machine in a confined space, wait until the arm retracts or confirm that both arms are located elsewhere. If you are trapped during extraction, wait for the arm to sink. On the final Machine, avoid completing it if an arm blocks your path, since you could lose time waiting for it to retract while the Panic Mode timer ticks down. You can try to bait an arm into attacking and then run past during its stun window. High Movement Speed helps here. Additionally, Treadmill Machine Skill Checks can lock you in place while arms spawn nearby, and there is little you can do to avoid damage in that scenario.

However, Twisted Blot is the only one who remains seated at all times, as the others stand during certain states such as chasing or enraging.
- His Research Capsules originally misspelled his name as "Blott," which has since been corrected. This is the second instance of a Research Capsule displaying an incorrect name, the first being Twisted Razzle & Dazzle's Capsules listing them as "RazzleDazzle."
- He is the second Twisted with uniquely shaped pupils, after Twisted Gigi, followed by Twisted Soulvester and Twisted Squirm. His pupil pattern matches the eye drawing of Blot Jr. Among these Twisteds, only he and Squirm lack star-shaped pupils.
- He is the second stationary Twisted alongside Twisted Razzle & Dazzle and Twisted Rodger, with Twisted Squirm being the third. All stationary Twisteds have unique walking and running animations in Roleplay Mode. For Blot and Razzle & Dazzle, these are nearly identical to their Toon counterparts' animations. Twisted Gourdy could also be considered stationary, though only in his passive state. Twisted Blot is the first non-Uncommon stationary Twisted, being Rare, with Twisted Squirm being the second.
- He is the fourth Twisted with a potential ranged attack, following Twisted Goob, Twisted Razzle & Dazzle, Twisted Rodger, and Twisted Scraps as the first, Twisted Sprout as the second, Twisted Gigi as the third, and Twisted Bassie as the fifth. Among these, Twisted Blot, Twisted Razzle & Dazzle, Twisted Rodger, and Twisted Sprout are the only ones whose ranged attacks cannot be blocked by Cardboard 'Armor.'
- He is the third Twisted with a trap-summoning ability, after Twisted Sprout and Twisted Bassie. His traps are quite similar to Twisted Sprout's, both being dark or Ichor-based structures that damage nearby Toons. This parallels his Toon counterpart's ability to summon entities.
- He is the first Twisted whose main body does not grant Research, Tapes, or Event Currencies on approach. This is because his detection range is stored in his hands, not his body. Twisted Gourdy shares this trait but only in his passive state.
- He is the fourth Twisted whose main body does not deal contact damage, after Twisted Razzle & Dazzle (non-hostile), Twisted Glisten (passive), and Twisted Connie (roaming), with Twisted Gourdy (passive) being the fifth. He is the only one among them with a single state, meaning his body is never directly dangerous.
- Before the Elevator opens, you can sometimes hear him summoning his hands if they are close enough, making him technically the second non-Main Twisted with audio cues at the start of a Floor, after Twisted Brightney. However, Brightney's cue only plays after the Elevator door opens, making Twisted Blot the only non-Main Character Twisted with a pre-entry audio cue.
- His hand traps are not highlighted by Brightney and Vee's Active Abilities or Party Popper. Only his main body gets highlighted.
- He is the third Twisted with optimized animations but no attack face, after Twisted Coal and Twisted Pebble, with Twisted Glisten being the fourth (not counting his enraged face).
- He has two distinct attack animations, one for each hand. His right hand performs a slam while his left hand executes a turning swipe. He is technically the second Twisted with multiple attack animations, after Twisted Scraps, who can damage Toons with both her tail and melee strikes.
- Due to the random nature of his hand placement, they can appear in unusual locations such as on top of crates, inside map exhibits, on Ichor Puddles, or even hovering above other stationary Twisteds like Twisted Razzle & Dazzle, Twisted Rodger, passive Twisted Gourdy, or even Twisted Blot himself. This produces unusual visuals like fingers clipping through walls or hands sinking into shelves, but the hands remain fully functional and dangerous.
- His predominantly black color palette makes both him and his hands very difficult to see during Blackouts, alongside other hard-to-spot elements like Twisted Finn, Twisted Toodles, Twisted Coal, Twisted Rodger's Capsule, and Twisted Sprout's tendrils.
- Occasionally when his hands retract, they may briefly snap upright before unloading, likely a momentary T-pose. The same can occur when they respawn.
- There is a known bug where his hands can attack players even while they are still in the process of emerging from the ground, potentially costing a Heart or causing death at low health.
- His hands function and look similar to Twisted Sprout's tendrils: dark, limb-like structures that strike when a player gets too close.
- The game files contain an unused animation called "T_Blot_LostInterest" showing him shaking his head similar to how Twisted Boxten and other Twisteds react when losing line of sight. It was likely left unused because of its limited applicability for a stationary Twisted.
- In one of Blot's interactions with Razzle & Dazzle, he mentions wearing 3 pairs of gloves to prevent ink from seeping through. Despite Twisted Blot's gloves being torn, no ink or Ichor is visible dripping directly from his hands, though they can sink into the ground.
- After the v0.16.1 release, many players reported that Twisted Blot had vanished from the game. Following patches v0.16.7 through v0.16.11, developer Qwel confirmed the disappearance and confirmed he had begun spawning again.
